Hours experience and/or apprentice hours that you have logged

Armed Forces Separation of Service
Exam Registration
Social Security Statement
Verification of Prior or out of state Licensure
Copy of temporary permit
Copy of student license (if not previously sent to the board office).

For some types of cosmetology licenses, the fee is required upfront. For others, the fee is paid after the State has reviewed your application.
You'll need to enter your credit/debit card information to pay the permit fee. We accept Visa and MasterCard, and ACH payments.
The fees for a cosmetology license exam are as follows:
$15 Cosmetologist Application Processing Fee
$48 Cosmetologist Per Year License Fee (2 years @ $24 per year)
